Pharmacy & Therapeutics Consulting offer an After Hours Order Review Service that ensures new or changed medication orders written while your pharmacy is closed are reviewed for appropriateness by a qualified clinical pharmacist.

We provide this service with three important goals in mind.
     Meet or exceed JCAHO standards

            the pharmacy is open resulting in continuity of care.
Typically our service is set up to seamlessly work with your current computerized medication order processing system via secure Internet communication tools. This is made possible using highly innovative communication technologies coordinated by our partner and affiliate The Center for Drug Safety, Inc. (CDS). CDS has designed a medication management system with full MICROMEDEX clinical screening and drug information resources that is a true application service provider (ASP). It is 100% server side allowing for the communication between systems to be easily set up.

With this unique communication tool our pharmacists can be trained to review medication orders from multiple facilities using one system. Differences among the various hospital systems are handled by the communication tool resulting in a medication order review process that is transparent to the end users at both P&TC and your facility. This innovative method uses the Internet in a safe and secure manner allowing our pharmacists to work from home thereby further reducing costs. (Alternatively we can set up a system to process your manual medication orders with full MICROMEDEX clinical screening or remotely access your facilities automated systems in a more traditional fashion using older communication technologies)

Our pharmacists will be trained with documentation of competency in your organization's clinical pharmacy programs, as are your pharmacists. This may include initiatives such as dosing protocols, IV to PO conversion, therapeutic interchange protocols, laboratory monitoring, dosing adjustments based on renal clearance and other pharmacy based clinical services. Additionally, our pharmacists would have the ability to utilize your automated products such as CPOE pending order lists, computerized order entry, interfacing with the laboratory system to evaluate pertinent values, clinical documentation tools, etc. via our innovative communication technologies.

Our goal is having the medication ordering system work exactly the same as when the pharmacy is open. This entire process will be transparent to the physicians, nurses, and other health care professionals who rely on pharmacist based clinical services.

If you are currently using non-pharmacists (such as nurses or physicians) to review medication orders please be aware that the 2006 JCAHO standards will most likely REQUIRE a PHARMACIST review of orders. How do we save you money?

Service rates are determined by the estimated amount of actual work time required to provide this service vs. the number of hours covered. Typically this saves up to 50% of the cost of an on-site pharmacist. We are prepared to provide any coverage needed to include 24/7/365.
